01Consent to Electronic Records and Signatures

You have the option to complete and sign documents, and to receive notices and other records relating to your request, electronically rather than on paper, through localfiberinternet.com (the "Site") in your dealings with P50 Digital LLC ("Company," "we," "us," or "our").

When you complete a form on the Site and click a consent, agreement, or submit button, you agree to affix your electronic signature to what you submitted. You acknowledge that your electronic signature carries the same legal effect as a handwritten one. You further agree to receive records and communications electronically and to execute documents electronically.

02What You Need

To access, complete, sign, and retain records electronically, you need:

• A personal email address you can access
• A computer, phone, or other device with an internet connection
• A current version of Chrome, Firefox, Edge, or Safari
• Software capable of receiving and reading email
• A printer or storage device, if you wish to keep your own copies

03Scope of Your Consent

Your consent covers:

• Records you complete, sign, or receive through the Site
• Records delivered to you by email
• Communications relating to the inquiry or request you submitted to us
• Communications from internet service providers, including AT&T®, and from alternative providers where AT&T service is not available at your address
• Requests for additional information needed to process your request
• Notices required by law, and notices of your rights under applicable law

Your consent applies to the current session and to all future sessions unless and until you withdraw it.

04Paper Copies

You are welcome to call us with questions about the services we offer. We do not provide non-electronic document execution. If you need paper copies or paper execution of provider documents, contact the underlying internet service provider directly to arrange it.

05Updating Your Information or Withdrawing Consent

If your contact information changes, or if you want to withdraw consent to electronic records and signatures, contact us at any time at privacy@p50digital.com.

If you withdraw consent:

• You will no longer be able to sign documents electronically or receive electronic disclosures from us;
• The withdrawal applies going forward only, and takes effect once we have had a reasonable opportunity to process it;
• You may need to contact the applicable provider directly to arrange paper delivery of its documents.

06Your Acknowledgment

By proceeding, you acknowledge that you:

• Consent to the use of electronic records and electronic signatures as described above;
• Consent to electronic storage of those records;
• Are able to access and retain electronic records.
